Application Submission Options
1) Fill out New Business Coversheet 2) Submit Application via Fax (866-470-9520) or email (nb@highestcontract.com) Exceptions: All applications submitted for US Financial must have the originals mailed to us as well. If we do not recieve the original signed copy with the premium check within 10 business days, we will cancel the submission. If you have collected a premium check from the applicant, please mail it to the above address. Do not send checks directly to the insurance company.
